Family Engagement Specialist

Position Description:
Develop and implement a Family Support and Acceptance Model Program that serves to prevent homelessness and expand the resources for LGBTQ youth to provide safe and welcoming permanent and stable housing.

FLSA Status: Full Time; Non Exempt

Reports to:  Director of Operations

Responsibilities:

Program Development

  • Research Family Support Model Projects and Host Home programs with guidance from Executive Director and Director of Operations; Identify prospective trainers and professional training conferences to obtain program models, curriculum, and skills-based training; schedule trainings / conferences in 2016-2017
  • Conduct needs assessment activities, including interviews and focus groups, with identified parents/ guardians of LGBTQ youth enrolled at JASMYN. Expand assessment to include community partners with expertise in family support and services, including PFLAG, Youth Crisis Center, and Daniel Memorial
  • Develop Program design for the Family Support and Acceptance Model at JASMYN
  • Develop program evaluation tools, and data collection tools and procedures

Program Implementation and Evaluation

  • Conduct family and youth risk assessments and service planning
  • Provide crisis intervention, suicide intervention and prevention to parents
  • Cultivate and support family engagement in educational groups
  • Provide education to families on LGBTQ youth issues and risks
  • Provide education on family acceptance interventions
  • Develop and co-facilitate group meetings for parents with community partners (PFLAG)
  • Develop and implement a Parent Night at JASMYN
  • Develop collaborations for training and resource development with community partners such as PFLAG, YCC, and Daniel Memorial
  • Assist JASMYN Homeless Services Case Manager in developing and locating alternative housing resources for homeless LGBTQ youth with community partners
  • Coordinate parent services with youth services case managers and medical and social service providers when and as appropriate
  • Implement evaluation and data tracking plan
  • Document and report all services according to all contract requirements in a timely manner
  • Participate in all staff and interagency meetings and trainings as required
  • Ensure participant confidentiality at all times

Qualifications:

  • BA or higher degree in human services field
  • 3-5 years’ experience working with youth
  • Extensive experience in providing family services, crisis intervention, parenting or therapeutic services
  • LGBT Cultural Competency

Case Manager

The primary role of the JASMYN Case Manager is to provide outreach, engagement and linkage casework services to high risk young men of color who have sex with men (YMCSM) as well as provide medical linkage and support services to newly diagnosed HIV positive youth, 13-23 years old, while also working with HIV positive youth to maintain their health and medical adherence. We are seeking to hire a qualified professional that has experience working with LGBT youth and youth that have multiple challenges and needs. This person will have experience working with youth development programs and HIV prevention and intervention.

Additionally, the Case Manager will provide supportive services for youth who are homeless, marginally housed, or at risk of homelessness, experiencing issues with substance use or abuse and with youth that have a lack of adult support and resources.  The Case Manager is responsible for intake and assessment; case planning and resource brokerage and linkage; case coordination and advocacy, crisis intervention and follow-up.

Qualifications/Skills

Ÿ Cultural Competency, including knowledge and sensitivity, with gay, lesbian, bisexual, transgender, questioning (LGBTQ) youth and other young men of color who have sex with men (YMCSM)

Ÿ  Minimum of Bachelors Degree

Ÿ  3-4 years working with youth or in a similar setting

Ÿ  Experience working with target population and HIV/AIDS

Ÿ  Case management experience

Ÿ  Background Screening

Ÿ  Drug Screening (Required by some organizations)

Ÿ  Current/Valid Florida drivers license with a safe driving record

Ÿ  Must have own transportation

Ÿ  Willing to work a flexible schedule (days and hours)

Ÿ  Willing to work with the target population, conducting outreach in the community as required

Ÿ  Must be resourceful, having a functional knowledge of area resources

Ÿ  Must understand and respect boundary issues in performing duties

Duties

Ÿ  Conduct both online and street outreach in an effective and engaging manner

Ÿ  Conduct rapid HIV testing including pre- and post-test counseling

Ÿ  Screen and assess candidates for hepatitis and substance abuse treatment, as indicated and appropriate

Ÿ  Engage and link individuals to services as appropriate

Ÿ  Link HIV positive clients to medical care and all other appropriate care and services

Ÿ  Support HIV positive clients in treatment retention and adherence

Ÿ  Provide risk reduction counseling to all program participants

Ÿ  Link HIV positive clients to all appropriate care and services

Ÿ  Provide and/or coordinate transportation services as needed

Ÿ  Coordinate services with other case managers and service providers when and as appropriate

Ÿ  Collaborate with other case managers and service providers as needed and appropriate

Ÿ  Document and report all services according to all requirements in a timely manner

Ÿ  Participate in all meetings and trainings as required

Ÿ  Ensure participant confidentiality at all times

Ÿ  Participate as a member of the JASMYN staff and service team developing JASMYN programs and services for high risk youth, including group facilitation, outreach and  enrollment, drop in center and clinic staffing

Ÿ  Other duties as assigned

Patient Navigator and Linkage Specialist

JASMYN’s Patient Navigator plays and Linkage Specialist plays key role in JASMYN’s HIV prevention, HIV testing and outreach and youth development programming. This position actively engages people living with HIV in enhancing entry into HIV care, promoting medication adherence, and facilitating retention in HIV care.

Qualifications:

  • BA in social work or a related field such as social work, social welfare, sociology, public health or psychology from an accredited university; preferred.
  • 1-3 years’ experience working with youth or in the social work field
  • Familiarity and sensitivity in dealing with the unique needs of LGBTQ youth
  • Experience with HIV prevention, testing, and health promotion programs
  • Ability to relate to teens and young adults in an accepting, positive and healthy manner
  • Ability to coordinate multiple responsibilities and prepare regular reports
  • Ability to develop and lead group and individual sessions with youth that have multiple needs.
  • Strong communication skills
  • Strong outreach skills
  • Strong assessment skills and ability to document and prioritize needs
  • Strong community organizing and outreach skills
  • Data management experience preferred, including familiarity with Excel or other data management software
  • Must be available to work flexible schedule that will include weekdays, most evenings and some weekends.
  • Must pass criminal background checks
  • Current FL driver’s license and clean driving record

Duties include:

  • Provide support and linkage services for HIV positive clients to medical care and all other appropriate care and services
  • Reengaging HIV positive youth lost to care
  • HIV testing
  • Community and street outreach
  • Support Case Managers with  psychosocial assessment and service planning
  • Support Case Managers with scheduling doctor appointments, assisting clients at doctor appointments and advocating for their care
  • Support Outreach efforts at community information fairs & conferences
  • Work closely with youth to assist them with their identified needs and internal linkages and community referrals; Provide supportive services to youth at the drop in center
  • Conduct outreach and enroll membership into JASMYN programs and services
  • Support and assist with intra-agency referrals and linkage
